Student Care
Welcome! Columbia has a committed staff of male &
female Clincal and Pastoral Counsellors available to students.
Applications for on or off campus counselling begin with the intake and
referral questionnaire. You may pick up a copy in person or
download the questionnaire below. Every request receives
individual, careful, and confidential attention from the
College's Student Care Office.
(NOTE: If you choose to download this copy rather
than pick up a questionnaire from our offices, please place it in
a sealed envelope marked "intake Counsellor: Student Care Centre" and hand it in at
the front office, Student Development offices, or Student Care &
Career Centre offices. Please be sure to sign your name at the bottom of the questionnaire.)
